Pathway Planning
Pathway Planning is a forward-thinking approach to transition planning and assessment that prioritizes the individual with a disability. Our skilled team works closely with you and your family to understand your goals, aspirations, and preferences.
Our model brings together experts from various fields, including behavior analysis, occupational therapy, speech and language, assistive technology, college and career counseling, vocational coaching, and more. This multidisciplinary team collaborates to gather and analyze data.
Using this insight, we create a customized Pathway Plan that outlines clear, actionable next steps aligned with the individual’s vision for the future and connects families with local resources and supports.
